Westside Youth Rugby
Parents and Players,
Welcome to the 2010 Westside Youth Rugby season. Whether you are new to rugby or are an experienced player, we will work to improve the skills and abilities of every player, so each player develops their full potential.
IN ORDER TO BEGIN PRACTICE, ALL REGISTRATION FORMS AND PAYMENT MUST BE COMPLETE AND RETURNED.
TEAM FEE: $80
• Fee includes TRYCO dues, USA Rugby registration membership dues, and team socks. Players new to Westside will receive a FREE rugby ball.
*All players must have a Westside Rugby jersey. You can wear a jersey from a previous year (if you have one) or you may purchase a new jersey for $25.
REGISTRATION FORM INSTRUCTIONS:
Please complete all forms including:
• Emergency Contact Information - Be certain you thoroughly read “Authorization to Consent to Medical Treatment for a Minor.” Signature of parent and athlete required.
• Code of Conduct – Read and discuss all requirements. Parent and Athlete initials are required for each of the four items, and the signature of and parent and athlete required.
• Waiver and Release of Liability – Read thoroughly. Signature of and Parent and Athlete are required.
• Order Form– Read the page entitled "Order Form Instructions" and fill out the Order Form thoroughly. Use the "Order Form" to determine your total payment. Payment can be by check or credit card. Checks should be made out to "Westside Youth Rugby". To pay by credit card, click the "Pay Now" button on our website. (Additional fees will be charged for all credit card orders.)

A PARENT’S and ATHLETE’S signature should appear three times.
Forms are computer friendly and text can be entered directly on the form and printed, or you may print them and complete them in clear hand-print. Forms and dues will be collected at the registration meeting or they can be mailed to the below address.
